Provider My name is Gybely Diaz-Gonzales and I am a Board-Certified Advanced Practice Registered Nurse with an Autonomous license to practice in the State of Florida. I received my Undergraduate degree in nursing from Walden University, followed by a Graduate degree in nursing, specializing as a Family Nurse Practitioner, from Purdue University. I hold certification as Family Nurse Practitioner from The American Academy of Nurse Practitioners. I have over 17 years of nursing experience with the last 4 years working as a Nurse Practitioner at a private family medicine practice. My scope of practice encompasses health promotion, disease prevention, diagnosis and management of common and complex health care problems including obesity.
